Case #4

 Patient:: a 68-year-old man

Chief complaint:
Sense of discomfort and swelling in the left side of the face (infraorbital area)

Recent medical history:
The patient underwent the extraction of the left first premolar because of the radicular lesion from the diagnosis made by the local dentist and then took the denture treatment on May in 2006. At that moment, there was nothing specific but the patient complained the pain and swelling in the site treated on April 19 in 2010. The doctor found the swelling on the gingival of the left premolar area. The doctor practiced the alveoplasty on that site of the patient and ordered him to take medicine. But the symptoms which the patient felt did not subside enough well. So he was referred to the local dental hospital in the impression of maxillary sinusitis. Because the doctor in that local dental hospital found that the patient needed to take more advanced diagnosis and treatment, he visited our hospital on April 28 in 2010.

Current condition of disease:
Initial examination showed the swelling, tenderness, and ulceration of the involved gingival and the swelling and tenderness on the left side of the middle face and nasal obstruction.

Family medical history:
The other member of the family: non-specific
